Download and print out the Entry Form (2 Pages) from your computer. Complete all questions, and submit the Entry form with your materials and fees. Electronic submissions will not be accepted. If there is more than one Entrant, each person needs to complete a separate form. Clip the ‘group’ Entry Forms together and submit with the required materials. If multiple category submissions are submitted in one envelope, make sure the Entry Forms and materials are correctly grouped together.
